Η διάγνωση του σπαστικόυ ραιβόκρανου, στηρίζεται στην κλινική εξέταση του ασθενούς που περιλαμβάνει λειτουργική και νευρολογική εκτίμηση και το ιστορικό του. Δεν υπάρχει κάποιο συγκεκριμένο test που να επιβεβαιώνει τη διάγνωση, ενώ ο εργαστηριακός έλεγχος είναι φυσιολογικός. Προσοχή πρέπει να δίνεται ώστε να μη γίνεται σύγχυση αυτής της κατάστασης με εκφυλιστικά προβλήματα στη περιοχή του αυχένα ή προβλήματα στα μάτια, όπου υπάρχει κλίση της κεφαλής για να βελτιώνεται η όραση.
